Description
A Registered Nurse (RN) is responsible for conducting in-person nursing assessments at the client’s home.
Responsibilities
- Conducts initial in-person nursing assessments of each client to establish a baseline of the client’s physical and functional status and identification of the level of services needed to meet the client’s needs. This includes, but is not limited to, the following:
- Record vital signs, including temperature, pulse, respiration, blood pressure, and a pain assessment
- Record past medical history
- Record the health needs of clients
- Listing of medications
- Assess functional status
- A psycho-social review
- A systems review
- Assess nutritional status
- A home safety review, including a fall-risk assessment
- Review of advance directives
- Conducts in-person re-evaluations of the client and the continued appropriateness of the client’s
Care Plan/Service Plan every 60 days, or more often. - Consult with Care Coordinator(s) to decide on the best Care Plan/Service Plan for clients
- Ensure service is delivered in accordance with Firm policies, procedures, and industry standards
- Attend orientation, in-service training sessions and staff meetings
Qualifications
- High school diploma or equivalent required
- Current Registered Nurse (RN) license is required
- Associate’s or Bachelor’s degree in Nursing is required
- Current First Aid and CPR certification is required
- Current driver’s license and proper vehicle insurance coverage
Experience
- Nursing: 1 year (required)
- Home Care: 1 year (required)
