Description

A Registered Nurse (RN) is responsible for conducting in-person nursing assessments at the client’s home.

Responsibilities

  • Conduct initial in-person nursing assessments of each client to establish a baseline of the client’s physical and functional status and identify the level of services needed to meet the client’s needs. This includes, but is not limited to, the following:
    • Record vital signs, including temperature, pulse, respiration, blood pressure, and a pain assessment
    • Record past medical history
    • Record the health needs of clients
    • Listing of medications
    • Assess functional status
    • A psycho-social review
    • A systems review
    • Assess nutritional status
    • A home safety review, including a fall-risk assessment
    • Review of advance directives
  • Conduct in-person re-evaluations of the client and the continued appropriateness of the client’s Care Plan/Service Plan every 60 days, or more often. 
  • Consult with Care Coordinator(s) to decide on the best Care Plan/Service Plan for clients.  
  • Ensure service is delivered in accordance with the firm’s policies, procedures, and industry standards.
  • Attend orientation, in-service training sessions, and staff meetings.

Qualifications

  • High school diploma or equivalent required
  • Current Registered Nurse (RN) license is required
  • Associate’s or Bachelor’s degree in Nursing is required
  • Current First Aid and CPR certification is required
  • Current driver’s license and proper vehicle insurance coverage

Experience

  • Nursing: 1 year (required)
  • Home Care: 1 year (required)
