Description

A Physical Therapy (PT) Consultant assesses, plans, organizes, and administers rehabilitative programs that improve mobility, relieve pain, increase strength, and improve/correct disabling conditions resulting from disease or injury; accomplished within the comfort of the clients’ home.

Responsibilities
  • Performs and documents an initial assessment, evaluating data to identify problems, and determining a diagnosis prior to intervention
  • Consults with clients to learn about their physical condition and symptoms and review theirmedical history and referrals from doctors/surgeons
  • Plans, prepares, or carries out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in clients
  • Records prognosis, treatment, response, and progress in clients’ file
  • Identifies and documents goals, anticipated progress, and plans for reevaluation
  • Administers manual exercises, massage, or traction to help relieve pain, increase patient strength, or decrease or prevent deformity or crippling
  • Evaluates effects of treatment at various stages and adjust treatments to achieve maximum benefit
  • Tests and measures clients’ strength, motor development and function, sensory perception, functional capacity, or respiratory or circulatory efficiency and record data
  • Instructs clients’, their families, and the assigned Care Assistant(s) in treatment procedures to be continued at home
  • Ensures service is delivered in accordance with Firm policies, procedures, and industry standards
  • Observes how the client’s functioning and report to the Care Coordinator
  • Attends orientation, in-service training sessions, and staff meetings
Qualifications
  • Current Physical Therapist (PT) license required
  • Bachelor’s degree or higher in Physical Therapy required
  • Current First Aid and CPR certification preferred
  • Must have reliable transportation
Experience
  • Physical Therapy: 1 year (preferred)
  • Home Care: 1 year (preferred)
